PTA Membership Form

INSTRUCTIONS:

** Deadline for PTA membership and Directory extended to Friday, September 19 **

• Please fill out the PTA membership form first and click "Submit".

• Then pay your Membership Dues, Donate and/or Order an Extra Directory on the right.

• Choose the type of membership desired and click on "Add to Cart".

• You will be transferred to a PayPal Shopping Cart website.

• Click on "Continue Shopping" to return to this page to choose a Sponsor amount and extra Directory if needed.

• When you are satisfied with what is in your cart, click on "View Cart".

• To donate larger amounts increase the quantity of items in your cart.

• You DO NOT need a PayPal account to pay. You can use a credit card if preferred.
